Working to support older people in the community

Age Concern North East Wales does the vast majority of its work out in the communities of Flintshire and Wrexham. We support older people to live independently in their own homes for as long as they choose to do so. We want them to have real choices over their own lives and feel safe, secure and be well informed when making any choices.

We offer two projects that do this, Wellcheck in Flintshire and Options in Wrexham. They provide advice, support and information to people over the age of 50 in their own homes who, without the project’s help, might soon need more intensive help from more formal services.

In both projects a team of advisors provides short-term, one to one support to help older people to maintain, or regain, their independence, good health and wellbeing. They work closely with the older person to identify the issues causing problems. They then use their local knowledge to put together a range of options to sort matters out. Some of the issues about which Wellcheck can supply information are:

  • Physical and Mental Health - including information on diet and nutrition, physical activity, emotional well-being and bereavement support;
  • Finance and Benefits - including information and support on accessing benefits advice and individual grant schemes etc;
  • Social Activities - combating loneliness, volunteering opportunities, education and learning, local social groups and social activity;
  • Transport - specialist and community transport schemes, public transport;
  • Carers - supporting older people who are in a caring role and identifying 'hidden' carers;
  • Home Maintenance and Security - including information about repairs, home security, crime prevention, and fire safety checks;

A referral to these projects can be made by anyone, so long as it is with the consent of the person whom they want to refer. The service is entirely free of charge and confidential. For further information, on Wellcheck please telephone us on 08450 549969 and for Options call 08450 549822.


Volunteer

We are looking for people in Flintshire and Wrexham to give up as little as an hour a week of their time to volunteer with Age Concern.
